Yes — gynecomastia surgery is permanent. Once the glandular tissue is surgically removed, it cannot regrow. The only exception is if the underlying hormonal cause (such as steroid use or certain medications) is not addressed and you continue using them after surgery. In that case, new glandular tissue could develop. For most men, one surgery = permanent results for life.
For Grade 1 and Grade 2 cases — which represent the majority of patients — we use the periareolar technique, placing a tiny incision at the lower border of the areola. This scar is virtually invisible once healed, as it blends into the natural colour boundary of the areola. We typically recommend waiting until 18 years of age, as hormonal fluctuations during puberty sometimes cause temporary gynecomastia that resolves on its own within 1–2 years. However, if gynecomastia has persisted for over 2 years past puberty, it will not resolve spontaneously. There is no upper age limit — men in their 40s, 50s, and 60s routinely undergo gynecomastia surgery with excellent results.
The key difference: gynecomastia feels like a firm, rubbery disc of tissue directly under the nipple — it's hard and defined. Chest fat (pseudogynecomastia) is soft, evenly distributed, and does not have a distinct lump. If you can feel a firm, slightly tender lump beneath one or both nipples that doesn't go away with exercise or diet, it is almost certainly true gynecomastia. A clinical examination confirms the diagnosis.
In adolescent males, gynecomastia sometimes resolves on its own within 6 months to 2 years as hormone levels stabilise. However, if it has persisted for more than 2 years, or if you are an adult with glandular tissue present, it will not go away without surgery. Medication has limited effectiveness and is not a long-term solution. Diet and exercise have no effect on true glandular gynecomastia — only surgery removes the gland permanently.
For Grade 1 and 2 cases, we use local anaesthesia with light sedation — you're comfortable and unaware, but not unconscious. Recovery is faster, and you can be discharged in 4–6 hours. For Grade 3–4 cases requiring skin excision or longer operating time, deep sedation or general anaesthesia is used with a short overnight stay.
